First you have to know how to design a database, and you have to know proper accounting practice. Then you have to talk to someone in the clinic or hospital. Every area of medicine does things differently. (The patient database in a large hospital wouldn't normally be on the same computer as the purchasing, AR, AP and GL database - but it might be in a small clinic.)
